There are several scenarios where your parish, school or other Archdiocese entity may require a certificate of insurance (COI), including the following:
1) Special Event Permit to the city or parish where your event occurs;
2) Use of equipment such as food trailers at a special event you are hosting;
3) Use of facilities owned by other entities, including athletic fields, sports arenas, swimming pools, parks, parking lots, event / meeting spaces, etc.;
4) Lease of equipment;
